Overview
Cagrilintide is a long-acting, synthetic, acylated amylin analog developed by Novo Nordisk for obesity treatment. Native amylin (islet amyloid polypeptide/IAPP) is a 37-amino-acid peptide co-secreted with insulin from pancreatic beta cells after meals. Cagrilintide overcomes native amylin's rapid degradation and aggregation tendency through amino acid substitutions and fatty acid acylation, extending its half-life to support weekly dosing. It activates amylin receptors (AMY1, AMY2, AMY3) to promote satiety, slow gastric emptying, and suppress glucagon. The CagriSema combination (cagrilintide + semaglutide) has demonstrated up to 22.7% mean body weight loss in Phase 2 trials.
